Description
Performs duties related to direct patient care. Schedules, plans, organizes and provides professional nursing services for patients of assigned physician(s) following established nursing practice standards and medical office and/or hospital procedures. Provides coordination of care longitudinally to patients with IBD. Focuses on achieving optimal outcomes and decreasing preventable ER and hospital admissions. Serves as a consistent and accessible point of contact for patients with IBD and works as a liaison between the patient and IBD care team.
Education: Graduate of an accredited school of nursing.
Licensure/certification: Current RN license to practice nursing in the State of Iowa. Certification in Basic Cardiac Life Support.
Previous Work Experience: Two years nursing experience in an acute care hospital setting required, specialty-specific experience preferred.
Essential Functions
Assist leadership in protocol development in support of current practices.
Demonstrates ability to consistently meet work schedule demands in the hospital and the clinic.
Utilizes the nursing process and effective written and verbal communications skills in the provision of safe, effective, high quality nursing services to patient populations. Delivers patient care consistent with established professional nursing standards and hospital/clinic policies and practices.
Identifies high risk patients with ongoing care needs and conducts targeted outreach to these patients to ensure timely and efficient care delivery
Utilizes risk assessment tools to identify patients with potential needs requiring care coordination.
Works collaboratively with the interdisciplinary team and provides ongoing communication with the patient, family, and IBDteam
Monitors patient compliance with plan of care
Utilizes and maintains patient list(s) for monitoring of patients on high risk medications
Provides ongoing IBDeducation to patients and nursing staff, including medication administration teaching and clinical updates
Works on quality and process improvement initiatives
Facilitates necessary outpatient care such as homecare and infusion center services, including prior authorization requests
Creates a caring and compassionate patient focused experience and improves patient satisfaction
Assists in the implementation and improvement of a specialized IBDClinic
Plans and organizes a yearly IBDeducation event for patients and nursing staff
Coordinates fundraising and promotion of annual Crohn's and Colitis Foundation walk
Assists in other GI clinic nurse roles as needed
Maintains awareness of own safety in carrying out duties and responsibilities. Adheres to universal precautions and infection control policies and procedures.
Assumes accountability for enhancing own knowledge through professional associations, journals, workshops, in services, etc.
Keeps professional license current.
Maintains patient confidentiality and uses discretion in patient interactions.
Promotes a positive, professional image of The Iowa Clinic both internally and externally. Promotes positive interpersonal relations.
