A Typical Day for Clinical Educator with Hearthstone!

• Complete chart audits and identify quality or care gaps

• Work with the Director of HO to identify a plan to address those gaps

• Work 1:1 with the nursing team in skills training and education to improve quality of care – end-of-life care, catheters, wound care, wound vacs, trachs, etc. Skills check-off as required to meet compliance

• Work with HCAs to complete skills check-off on hire and annually to meet compliance

• Partner with the Director of Quality on updates with COVID and how to train the team on new policies and procedures

• Participates in new hire orientation for home hospice staff

• Carry a small caseload of 5 to 7 patients week

• 70/30 In the field/in the office

• CNA class instruction
