We are seeking a compassionate, dedicated, and experienced Cardiac Rehab Dietitian to join our dynamic cardiac rehabilitation team. The ideal candidate will have a strong background in nutrition therapy, particularly in the areas of cardiovascular health, and a passion for promoting wellness through evidence-based dietary strategies. As a key member of the cardiac rehab team, you will provide expert nutrition counseling to patients recovering from heart disease, helping them adopt heart-healthy lifestyle changes to improve their overall health and quality of life.

Key Responsibilities:

Nutrition Assessment & Counseling : Conduct comprehensive nutrition assessments for cardiac patients and develop individualized dietary plans which emphasizes heart health.

Patient Education : Provide one-on-one counseling and group education sessions on heart-healthy eating, weight management, and lifestyle modifications that support long-term cardiac health.

Collaboration with Medical Team : Work closely with physicians, nurses, exercise physiologists, and other healthcare providers in the cardiac rehab program to ensure a holistic, coordinated approach to patient care.

Progress Monitoring: Track patient progress through follow-up assessments, adjust nutritional recommendations as needed, and provide ongoing support to help patients meet their health goals.

Program Implementation : Guide patients in implementing the principles, including a heart healthy diet, regular physical activity, and stress management techniques.

Documentation & Reporting : Maintain accurate, up-to-date patient records and progress notes in compliance with regulatory standards and organizational policies.

Community Outreach : Participate in community wellness initiatives and provide education on the importance of cardiovascular health through workshops, presentations, and events.

Department: Cardiac Rehab

Purpose: Utilizes in-depth knowledge of dietetic practice that views patient care as a whole and understands care needs based on previous experience. Recognizes changes that require interventions and acts with speed and flexibility. The Dietitian monitors and evaluates the nutrition care plan, determines when any dietary issues have been resolved and makes additional recommendations for improvement in nutritional status. Develops effective relationships with other caregivers and provides leadership within the health care team to formulate integrated approaches to nutrition care process.

Report To: Supervisor, Cardiac Rehab

Supervisory Responsibility: Guidance: The job requires the provision of occasional guidance and training to others. The job does not have formal or official supervisory responsibilities.

Materials Responsibility: Very Limited. Work requires very limited responsibility for material resources. Examples of resources could include personal work materials, supplies or equipment, or very small amounts of cash. The employee has a very limited or indirect amount of control over these resources. Although human error might require the repair or replacement of materials, usually the cost of correcting these errors is minor. The variety and volume of resources is also very limited. Problems associated with material resources are very uncomplicated.

POSITION SPECIFICATION

Education: Bachelor's Degree

Field Of Study: Nutrition, or Dietetics

Special Training:

Training Preferred: Board Certified Specialist or Master's Degree; Basic Life Support;

Licensure/Registration: Licensed Dietitian, Registered Dietitian

Experience: More than 1 year experience preferred.

Interpersonal Skills: Interaction is with a variety of people inside or outside the organization. Communications are of moderate difficulty and sensitivity. Contact with others may involve detailed & lengthy dialogues & exchanges of information. Requires a moderate amount of interpersonal skills. Interactions involve dealing with moderately complicated problem situations or stressful encounters.

Working Conditions: There is very limited exposure to adverse environmental conditions. Some undesirable or unpleasant environmental characteristics may occur but the physical environment is generally safe and there is minimal health risk. No safety equipment or unusual precautions are required. The amount of time the employee may experience these minor adverse conditions would be limited to 10% or less of the work day.
